AbstractBackgroundThere is a paucity of information regarding the efficacy and safety of orlistat (Xenical). We evaluated and compare orlistat in a cohort of obese patients who received a placebo or an oral orlistat tablet. We enrolled patients on a random number basis, with a mean age of 60.6 years (range: 25–90 years), with and without diabetes mellitus and with diabetes treated with metformin or oral antidiabetic agents. We measured and evaluated the efficacy and safety of orlistat in patients with impaired glucose tolerance (IGT). We analyzed the data on orlistat use as primary endpoint (mean ± standard deviation). The pooled analyses of OR and OR+OR and OR and OR+OR+OR showed a significant difference between and orlistat groups for the primary endpoint. The results were consistent with those obtained in prior studies of patients with IGT. Orlistat reduced the fasting plasma glucose (FPG) level (p = 0.0001) but not the fasting blood urea and creatinine (p = 0.25) in patients with IGT. The results of this study support the conclusion that, despite higher fasting plasma glucose (FPG) levels, patients with IGT are still susceptible to the side effects of orlistat, while patients with IGT are more sensitive to these side effects. This finding suggests that patients with IGT who receive orlistat should be monitored for signs of side effects.
